This is our administrative base. We do not run clinics from this location.

Our lines are open Monday-Friday 8am-6pm

We close our lines for one hour between 11am-12pm every Thursday, and cannot respond to emails within this hour.

Your Records

Accessing your record

If you wish you access copies of your record in the Wellbeing Service, please email the records management team at RecordsManagement@spft.nhs.uk. Alternatively you can contact us by post to the address above. Please be aware that we will require a signed form of authority to ensure that we share information only with your informed consent.

What you need to know

As an NHS service, we keep information about you and your care to ensure that we can give you the best possible service. We may use this information to:

  • Make sure your care is of a high standard

  • Plan and develop services

  • Check how well we are performing

  • Help train staff and support research

  • Fund your care

Confidentiality and data protection

By law:

  • Everyone working with and in the NHS and social services must keep your information confidential

  • Everyone must keep your information secure and share it only when necessary

  • Wherever possible we will keep your information anonymous so that it cannot be linked to you

Your Feedback

How are we doing?

We like to know if you receive a particularly good service from us or if our service doesn't reach a high standard. We also welcome your feedback via our assessment and end of treatment patient surveys. You will receive links to these via text message.

How to make a complaint

If you would like to raise concerns about any aspect of your care, you can contact the Wellbeing Service directly via the address, email or phone number above. We will reply within 3 days and will aim to come to a resolution with you within 20 days.

Alternatively, if you wish to file a formal complaint, you can contact Sussex Partnership’s Patient Advice and Liaison Service at pals@spft.nhs.uk.

If you feel that you need some support with your complaint, you can contact POhWER by calling 0300 456 2370, emailing pohwer@pohwer.net or writing to PO Box 14043, Birmingham, B6 9BL. You can also send the word ’pohwer’ with your name and number to 81025.
